Omnia Community Health in Victoria is seeking a Grade 2 Speech Pathologist to deliver assessment and intervention for adults with diverse needs across center-based, telehealth, home visits and community settings.
You'll coordinate client care with families, carers and health professionals, supervise junior clinicians and students, and contribute to service improvement within a committed not-for-profit team.
Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health. Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.
We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.
We are committed to enhancing access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.
The role provides high-quality speech pathology assessment, intervention, and education services to adults from diverse backgrounds through a range of delivery methods, including center-based appointments, telehealth, home visits, and community-based services. Working with clients funded through CHSP, HACC, NDIS, and Home Care Packages, you will support individuals with a variety of communication, swallowing, neurological, and degenerative conditions, including conducting mealtime assessments and developing tailored management strategies. You will play a key role in coordinating client care through collaboration with families, carers, healthcare providers, and community supports, while preparing clinical reports and recommendations as required.
As a Grade 2 Speech Pathologist, you will provide assessment and intervention services to adults with diverse and often complex clinical presentations. You will play a key role in supervising junior clinicians and students, fostering multidisciplinary partnerships, and driving continuous improvement and innovation across the service.
Our benefits enhance everyday wellbeing and encourage you to take time to make a difference in the community:
