Job summary

SA Health's Southern Adelaide Local Health Network – Flinders Medical Centre invites a Social Worker to join Women’s & Children’s Allied Health. The role covers inpatient and outpatient social work services across the Women’s & Children’s division.

Reporting to the Disciplinary Lead Social Worker, duties include assessment, discharge planning, and collaboration with families and community agencies. Part-time, 22.5 hours/week with flexible working arrangements.

Qualifications

  • Degree in Social Work eligible for full membership of the Australian Association of Social Workers.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Strong organisational skills and ability to manage fluctuating workloads.
  • High level interpersonal and written communication skills with diverse staff and clients.
  • Ability to analyse problems and implement practical, client-centred solutions.
  • Articulate a Social Work perspective on client and system issues.

Responsibilities

  • Provide high quality social work care to allocated patients and engage families in care planning and discharge planning.
  • Ensure active participation of patients and families in assessments and early discharge planning.
  • Apply evidence-based social work knowledge to develop management and therapy plans.
  • Provide social work input to educational programs within Women’s and Children’s Division.
  • Support and supervise AHA staff and students in the work area.

Are you a Social Worker looking to make a difference in Southern Adelaide's local community?

Embedded within the Southern Adelaide Local Health Network (SALHN), you will work alongside multidisciplinary teams to provide high-quality social work services across Women and Children's inpatient and outpatient settings.

About the role:

Reporting to the Disciplinary Lead Social Worker (Women & Children), this role's responsibilities include assessment, consultation, intervention and review, education, liaison with community agencies, organisation of appropriate resources and management of discharge.

Duties for the role will include:

  • Provide high quality, safe social worker care to allocated patients, engaging patients and their families or carers actively in care planning and goal setting to ensure optimal outcomes.
  • Ensure active participation of patients and their families/ carers in their assessment and early discharge planning
  • Apply current evidence-based knowledge and skills to provide individualised or group-based care, to referred women, babies and children and their families. Care will involve comprehensive social work assessment and development of management and therapy plans and ultimately discharge planning.
  • Providing social worker advice and expertise to educational programs within Women’s and Children’s Division.
  • Supporting and supervising AHA staff and students allocated to the work area to ensure high quality, safe and timely service delivery.
  • Engaging in continuing education and self-development through active participation in professional development courses, in-service education programs, professional meetings and clinical supervision.
  • Contribute to the development, implementation and review of clinical policies, procedures, and guidelines relevant to care of women and children as directed by Discipline Lead Social Worker and AH Manager.
  • Actively participate in team activities and contribute to building a positive team culture aligned with the Women’s & Children’s Allied Health Team Charter, SALHN vision, and strategic plan.

About you:

  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
  • Ability to work independently and as a member of a multi-disciplinary team.
  • Demonstrated organisational skills including the ability to adapt to fluctuating workloads.
  • High level interpersonal skills, including conflict resolutions skills and the ability to communicate with patients, professional staff of different disciplines, administrative staff and staff of other agencies both orally and in writing.
  • Ability to exercise initiative, analyse complex problems and implement practical solutions.
  • Ability to articulate a Social Work perspective on client and systems issues.
