Queensland Government's Mackay Base Hospital Social Work Department seeks a Social Worker to join on a permanent basis. You will deliver psychosocial support, counselling, and care coordination within multidisciplinary teams and across hospital and community settings.

You will work with patients and families, participate in planning and service delivery, and contribute to education and professional development opportunities offered by the Health Service.

Responsibilities

Role detailsThe Social Work Department at the Mackay Base Hospital is seeking applications for a Social Worker to join their team on a permanent basis.For information on how to apply and the mandatory requirements for this role, please refer to the attached Role Description.Your opportunityCompetently deliver services independently or within a multidisciplinary team to consumer groups within the district, as determined through the Health Service Plan and priorities.About the TeamThe Social Work Department forms part of the Allied Health and Clinical Support Service Stream delivering a range of high-quality psychosocial services to consumers whose medical care and treatment is being provided through the Service Streams of: Lifespan and Critical Care; and Perioperative, Women's Health and Ambulatory Care; as well as to their families and caregivers.Our Social Work Service delivers care to patients, families and carers throughout our hospital system as well as in our community and rural based health services. Our Social Workers are members of the treating team and provide a broad range of generalist and specialist services including, counselling, care coordination and guidance regarding: diagnosis and illness management, treatment and recovery processes and referral information.About our Hospital and Health ServiceMackay Hospital and Health Service has 6 hospitals, 2 multi-purpose health services and 4 community health centres that employ more than 3,500 staff.
