Job summary

Bairnsdale Regional Health Service (BRHS) invites applications for an experienced Social Worker to contribute to patient care in Gippsland - East Gippsland. The role requires a Bachelor of Social Work and at least 3 years of experience in health settings, with skills in psychosocial assessment, counselling and discharge planning.

Qualifications

• A recognised Bachelor Degree (or higher) in Social Work.
• Minimum 3 years' experience in social work, preferably within a health or hospital setting.
• Strong skills in psychosocial assessment, counselling, crisis intervention and discharge planning.
• Excellent communication, collaboration and organisational skills, with the ability to work effectively within multidisciplinary teams.

Responsibilities

• Provide psychosocial assessment and discharge planning to patients and families.
• Deliver counselling and crisis intervention as part of a multidisciplinary team.
• Collaborate with health professionals to support patient outcomes and smooth transitions.
• Maintain clear documentation and effective communication with teams and families.

The Contract: Ongoing, Full Time, 80 hours per fortnight with a monthly ADO.

The ideal applicant will demonstrate:

• Strong skills in psychosocial assessment, counselling, crisis intervention and discharge planning.
