Job summary

SA Health is seeking a Senior Social Worker to join the Palliative Care service at Flinders Medical Centre. The role focuses on specialist psychosocial support for patients and families facing life-limiting illness, while mentoring staff in a multidisciplinary team.

Reporting to the Manager, you will lead development, implementation, coordination and evaluation of quality Social Work services for Palliative Care patients. Leadership, clinical expertise and person-centered care are essential.

Qualifications

• Degree in Social Work with eligibility for membership of the Australian Association of Social Workers.
• Extensive knowledge of Social Work assessment and therapeutic interventions for clients with complex needs.
• Ability to analyse clinical care issues to support quality activities, service evaluation or research.
• Demonstrated ability to establish networks with diverse clients, families and stakeholders in an interdisciplinary setting.
• Experience working in community settings and in palliative care settings.
• Experience in programme evaluation and service development within Senior Social Work.

Responsibilities

• Independently assess clients' abilities, problems and needs using formal and informal tools.
• Design and implement evidence-based interventions tailored to diverse populations.
• Drive interdisciplinary planning to enhance service delivery in Palliative Care.

About therole:

Reporting to the Manager, thisrole is responsible forthe development, implementation,coordination and evaluation of quality Social Work services forpatients of the Palliative Care Service.

Duties for the role willinclude:

• Independently assess clients'abilities, problems and needs using a variety of formal andinformal assessments as needed,
• Designing and implementingevidence-based intervention programs tailored to diverse clientpopulations,
• Drive interdisciplinaryclinical and strategic planning initiatives to enhance theoverall service provided to clients,
• Documenting assessmentfindings and interventions in accordance with servicestandards,
• Providing specialised clientand family education about level of functioning and ongoingmanagement,
• Developing, implementing andoverseeing systems for monitoring, reviewing and improving thestandards of services to clients in the Palliative Careprogramme and in other programmes as requested,
• Initiating, developing andcontributing to educational activities within theteam

Aboutyou:

• A degree or equivalent inSocial Work, entitling membership of the Australian Associationof Social Workers,
• Demonstrated high levelcompetency and extensive knowledge in Social Work assessmentand therapeutic interventions to clients with complex needswith a broad range of Senior Social Worker clinicalskills,
• Ability to critically analyseor evaluate issues related to clinical care in social work in amanner which can be applied to quality activities, serviceevaluation or research,
• Demonstrated ability toestablish and maintain effective networks and partnerships witha broad range of culturally diverse clients and stakeholders,including patients and their families, community services /agencies and other service providers, within aninterdisciplinary setting,
• Experience working in acommunity setting,
• Experience working in apalliative care setting,
• Experience in programevaluation, service development, combined with a broad base ofexperience in Senior Social Work practice
