SA Health invites applications for Senior Social Worker – Complex Transitions at Flinders Medical Centre/Noarlunga Health Service. The role coordinates complex transitions for patients under 65, emphasising NDIS and discharge planning within acute settings.

The incumbent provides expert clinical support and leadership to the Social Work Department, MDT and hospital executives, driving timely, safe outcomes for complex cases.

Qualifications

• Degree or equivalent eligible for full membership of the Australian Association of Social Workers
• Extensive experience with patients and families in Acute/Sub Acute Care setting
• Strong interpersonal and collaboration skills with multidisciplinary teams
• High level of oral and written communication; able to develop procedures and reports
• Commitment to person-centred, evidence-based and culturally safe practice
• NDIS experience desirable

Responsabilités

• Conduct psycho-social assessments and intervene with patients with complex needs
• Collaborate with the multidisciplinary team, external stakeholders, patients and families for timely, safe outcomes
• Discharge planning for patients with complex biopsychosocial factors, escalating barriers as needed
• Develop, improve and maintain services for early identification of long-stay patients in the acute hospital
• Provide expert clinical support, leadership, education and consultation re: complex discharge issues, NDIS, and community services
• Advocate for access to appropriate community services

Senior Social Worker – Complex Transitions Consultant

Senior Social Worker contract at Flinders Medical Centre and Noarlunga Health Service coordinating complex transitions for patients under 65, with a strong focus on NDIS. Contract until 2028.

About the role

The Social Worker operates under limited supervision in a specialist professional capacity in an interdisciplinary team. The incumbent is responsible to the Manager of the Social Work and Counselling Service for the delivery of high-quality services to patients on acute wards. Join our supportive and collaborative team in;

• Conducting psycho-social assessments and intervening with patients who have complex needs, utilising a range of evidence-based social work skills and proactive problem solving.
• Working collaboratively with the multi-disciplinary team, external stakeholders, patients and their families to achieve timely, safe and effective outcomes for patients.
• Discharge planning for patients with complex biopsychosocial factors, including high-level escalation of barriers.
• Developing, improving and maintaining services for the early identification of long-stay patients in the acute hospital
• Providing expert clinical support, leadership, education and consultation to the Social Work Department, MDT, and Hospital Executives re: management of complex discharge issues, NDIS, and other systems
• Advocating for access to appropriate community services.

About you

• Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
• Extensive experience working with patients and families as a social worker in a specialty area within the Acute /Sub Acute Care Setting
• Demonstrated well-developed interpersonal skills that foster trust, respect, confidence and cooperation with colleagues, patients, families, multi-disciplinary team members, external stakeholders and, senior staff
• Demonstrated high level of oral and written communications skills and the ability to develop written procedures, reports, proposals and presentations
• Commitment to person-centred, evidence-based and culturally safe practice
• Extensive experience in providing clinical management to complex and difficult casework
• NDIS experience is desirable
