Job summary

Metro North Health is seeking an Advanced Carer Peer Worker to provide peer support within the MH HiTH service at The Prince Charles Hospital, drawing on lived experience to support recovery and wellbeing for consumers and their families.

You will collaborate with clinicians and community organisations, advocate for consumers and carers, and contribute to education, health promotion, and service improvement, helping to deliver recovery-oriented care across the service.

Responsibilities

• Provide peer support and share lived experience to promote recovery, wellbeing and positive outcomes for consumers receiving Hospital in the Home services.
• Support consumers to identify strengths, develop recovery goals and participate in recovery and safety planning.
• Advocate for consumers, families and carers while providing practical assistance and support throughout the recovery journey.
• Collaborate with multidisciplinary teams and external organisations to promote recovery-oriented, person-centred care.
• Deliver and support lived experience education, health promotion activities and service improvement initiatives within MH HiTH.
