Darling Downs Health in rural Queensland seeks an Aboriginal and Torres Strait Islander Senior Health Practitioner to lead culturally responsive primary health care initiatives and guide teams to improve community health outcomes.

You will mentor a multidisciplinary workforce, partner with community members and health professionals, and drive quality improvement that strengthens services and patient experiences across our region.

Qualifications

• Current registration as an Aboriginal and Torres Strait Islander Health Practitioner.
• Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care Practice.
• Diploma in Aboriginal and Torres Strait Islander Primary Health Care.
• Demonstrated experience leading health programs, managing teams and achieving service outcomes.
• Strong communication, relationship-building and problem-solving skills in community and healthcare settings.

Responsabilités

• Lead high-quality primary health care programs that improve access, continuity of care and community health outcomes.
• Support and mentor a multidisciplinary team to achieve service goals, uphold standards and deliver safe, culturally responsive care.
• Partner with community members, health professionals and stakeholders to develop practical solutions to complex health needs.
• Drive quality improvement initiatives that strengthen services and support better experiences for patients and families.
