Job summary

SA Health's Barossa Hills Fleurieu Local Health Network is seeking a Senior Clinician (Occupational Therapist or Social Worker) to provide clinical leadership across a multidisciplinary team. You will manage complex patient journeys and help transitions between hospital, community, aged care and disability services across the Barossa, Hills and Fleurieu region.

Flexible work arrangements include travel across BHFLHN with hybrid/remote options where available; permanent, full-time and part-time

Qualifications

• Experience with complex health, disability and aged care needs.
• Proven ability to provide clinical leadership and mentoring to teams.
• Ability to coordinate services across hospital, community, aged care and disability sectors.
• Willingness to travel across the Barossa Hills Fleurieu region.

Responsibilities

• Lead the coordination of complex patient journeys and safe transitions between services.
• Conduct comprehensive assessments and develop tailored care plans promoting independence.
• Provide expert clinical advice and supervision to allied health and multidisciplinary teams.
• Foster partnerships to improve care coordination and patient outcomes.
• Contribute to service improvement and innovative care models.
