Job summary

The Salvation Army Australia is seeking a Senior Case Manager to provide advanced case management support for vulnerable young people in Leongatha, VIC. This permanent, full-time role is on-site and reports to the Outer South Program Manager, delivering specialist guidance and ensuring safe, effective interventions.

You will work within a trauma-informed framework, support the Journey to Independence Model of Care, and contribute to stronger outcomes across Gippsland.

Qualifications

  • Tertiary qualification in Social Work or related field.
  • Minimum three years' experience in social services, youth sector preferred.
  • Knowledge of risk, screening and supervision processes.
  • Current Employee Working with Children Check.

Responsibilities

  • Provide specialist case management and consultation for young people with a range of support needs.
  • Demonstrate a trauma-informed, psychologically informed environment (PIE).
  • Ensure supports align with the Journey to Independence Model of Care.
  • Be a member of the Outer South leadership team to ensure risk, safety and governance compliance.

About The Role

We are seeking a dedicated Senior Case Manager to provide advanced case management support, specialist practice guidance, and quality assurance expertise. You will be working with young people facing complex challenges, including homelessness, family violence, mental health concerns, alcohol and other drug (AOD) issues, and family breakdown. Through intensive, therapeutic, and evidence-informed interventions, you will empower young people to build resilience, achieve positive outcomes, and create pathways toward stability, wellbeing, and independence.


Reporting to the Outer South Program Manager, this is a permanent full-time position, based on-site in Leongatha, VIC.



Conditions

Compensation is in accordance with the SCHADS Award Level 6.



How You Will Make An Impact


  • Provide specialist case management and consultation for young people with a range of support needs

  • Demonstrate a strong commitment to working within a trauma and psychologically informed environment (PIE)

  • Ensure support provided to young people is strengths-based and aligned with the Salvation Army Youth Stream’s Journey to Independence Model of Care

  • Work as a member of the Outer South leadership team to ensure the Leongatha youth services team practice is compliant with expectations around risk, safety and governance

  • Support staff through expert guidance on complex cases, risk management, and critical incidents

  • Collaborate with key stakeholders to strengthen service integration and outcomes for vulnerable young people across the Gippsland region



What You Will Bring


  • Tertiary qualification in Social Work, Welfare, Community Development or related field

  • A minimum of three years' experience in a social service environment preferably in the youth sector

  • Working knowledge of the challenges, risk, screening process and supervision

  • A current Employee Working with Children Check

  • A current VIC Driver's Licence
