Job summary

Australian Health Elevation Services – Penrith NSW is seeking a Senior Community Worker to lead a high-intensity caseload within NDIS and Home Care Packages. The role demands expert case management, crisis intervention, and strategic service planning to achieve outcomes for complex clients.

You will mentor junior staff, facilitate cross-disciplinary collaboration, and ensure compliance with price guides and legislation while driving continuous quality improvement.

Qualifications

• Minimum 1 year of full-time experience in high-complexity community work, support coordination, or case management.
• Proven experience with NDIS and HCP clients and independent coordination.
• Qualification in Community Services, Social Work, Psychology, or Welfare.

Responsibilities

• Conduct biopsychosocial assessments for clients with complex needs.
• Develop and review highly individualised support plans addressing risk and disadvantage.
• Manage multiple high-risk cases and coordinate across NDIS/HCP funding streams.
• Lead multidisciplinary collaboration with health and housing professionals and agencies.
• Mentor junior staff and contribute to quality improvement and policy development.
