Job summary

Bolton Clarke is seeking a Care Manager to oversee clinical governance, staff management, change management, AN-ACC and resident care across our Greater Western Sydney homes.

The successful candidate will supervise RNs and PCWs, ensure compliance with standards, and drive continuous improvement while delivering high quality, personalised care to residents in a supportive team environment.

Qualifications

• Current Nursing Registration with AHPRA.
• Experience within Aged Care Sector as a Senior Registered Nurse or Clinical Nurse Leader (desirable).
• Willingness to obtain police and/or NDIS Worker Check prior to commencement.

Responsibilities

• Commitment to delivering high quality, personalised care to residents.
• Supervise and mentor RNs and PCWs; lead staff across governance and change management.
• Ensure compliance with aged care standards and audits; support continuous improvement.
• Develop, monitor and implement nurse care plans; assess, plan and evaluate care.
• Participate in clinical governance and AN-ACC activities.
