Job summary

Hunter New England Local Health District is seeking a committed Community Health Registered Nurse for Singleton Community Health. You will deliver care across clinic and home settings, including immunisations, health assessments and chronic disease management to rural communities.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Current unrestricted NSW Driver Licence.
• Accredited Nurse Immuniser (NSW), or plan to complete within 12 months.
• Experience in community or primary healthcare nursing.
• Ability to work independently with a diverse caseload.

Responsibilities

• Deliver community nursing services across clinic and home-based settings.
• Provide immunisation services in accordance with NSW Health guidelines.
• Undertake comprehensive health assessments and develop person-centred care plans.
• Deliver chronic disease management and preventative health programs.
• Coordinate care and facilitate referrals to appropriate health and community services.
• Maintain accurate clinical documentation and contribute to continuous quality improvement.
