Job summary

Healthcare Australia is seeking a Virtual Triage Nurse to conduct comprehensive telehealth assessments, prioritise patient needs by urgency, and coordinate appropriate care pathways. You will document histories and care plans in EHRs and work with GPs, nurses, and specialists to support timely referrals and treatment.

The role requires RN with AHPRA, 5+ years in primary care or triage, and strong communication in virtual settings.

Qualifications

• Registered Nurse with current AHPRA registration
• Minimum of 5 years’ clinical experience in primary care, triage, assessment or case management
• Experience in immigration detention or similar environments is a plus

Job description

As a Virtual Triage Nurse, you will be responsible for conducting comprehensive clinical assessments via telehealth platforms, prioritising patient care based on clinical urgency, risk, and presenting needs. You will play a key role in coordinating appropriate care pathways, ensuring patients are directed to the right service at the right time.

You will also provide patient education, support self-management strategies, and contribute to positive health outcomes through effective communication and clinical guidance.

Day to day!

• Conduct virtual clinical assessments, prioritising patients based on urgency, risk factors, and presenting symptoms.
• Obtain and accurately document patient histories, clinical information, and care plans within electronic health records.
• Coordinate referrals, diagnostics, and care pathways in collaboration with GPs, Nurse Practitioners, specialists, and allied health providers.
• Provide patient education and follow-up to support chronic disease management, self-management, and continuity of care.
• Work collaboratively within a multidisciplinary virtual care team while contributing to quality improvement and safe, compliant clinical practice.

Lets talk about you!

You are a confident and experienced Registered Nurse with a Bachelor of Nursing (or equivalent) and current unrestricted AHPRA registration, bringing a minimum of 5 years’ clinical experience in primary care, triage, assessment, or case management. You are highly autonomous, thrive in fast-paced virtual environments, and demonstrate strong clinical judgement, communication, and interpersonal skills, with proficiency in electronic health records and telehealth platforms. You also bring valuable experience in primary health, community or urgent care settings, with exposure to complex and chronic disease management, and ideally have experience working in immigration detention or similar environments.
