Job summary

Bidgerdii Community Health Services is seeking an experienced Registered Nurse to lead the RHD portfolio within a primary health care team. The role combines clinic-based nursing, immunisation delivery and chronic disease management across Rockhampton, Mount Morgan and Blackwater sites.

The ideal candidate will have current AHPRA registration, at least two years in general practice or primary health care, a recent immunisation qualification and experience with venepuncture and pathology

Qualifications

• Two years of recent experience in general practice or primary health care.
• Current AHPRA registration as a Registered Nurse.
• Recent immunisation qualification with experience vaccinating.
• Experience in venepuncture and pathology collection.
• Good knowledge of vaccine management and cold chain requirements.
• Experience in chronic disease management including care planning and follow-up.
• Experience undertaking comprehensive health assessments, including Aboriginal and Torres Strait Islander health assessments.

Responsibilities

• Lead the RHD portfolio within the nursing team.
• Clinic-based role including immunisation and chronic disease care.
• Perform clinical assessments and escalate abnormal findings.
• Coordinate follow-up and liaise with GPs and the broader clinical team.
• Travel across Bidgerdii sites as required.
