Job summary

Palm Lake Care seeks a Clinical Coordinator - Admission & Assessments. You will lead foundational assessments, ensure accurate documentation, and develop individual care plans for residents. You will coordinate AN-ACC reviews and funding through My Aged Care, balancing care needs with resident choice.

You will travel to other Palm Lake communities, mentor staff, and drive standards in documentation. AHPRA-registered RNs with ANACC experience are encouraged to apply.

Qualifications

• Registered Nurse qualification with current AHPRA registration.
• Demonstrated knowledge of ANACC, including assessments, claims submissions and workings within policies.
• Proven knowledge of current trends in nursing and aged care.
• Understanding of Aged Care Quality Standards and relation with ANACC.
• Experience with funding rules and ANACC validations.
• Interpersonal and negotiation skills with older people, GPs, and healthcare providers.

Responsibilities

• Foundational assessment and quality documentation of resident needs.
• Develop individual care plans and coordinate supporting referrals.
• Manage ANACC reviews through My Aged Care to ensure funding.
• Communicate with clinical staff, management and residents to ensure quality documentation.
