Job description

Clinical Care Coordinator - Registered Nurse

Position Available: Clinical Care Coordinator - Registered Nurse

Contract: Full-Time

Are you a Registered Nurse looking for an opportunity to use your clinical expertise in a different and rewarding setting?

Enabling Pathways is seeking a skilled and compassionate Clinical Care Coordinator - Registered Nurse to strengthen clinical oversight across our disability support services in Cairns.

This is an opportunity to combine your nursing knowledge with staff training, clinical quality assurance and hands-on support for participants with complex health and care needs.

You will work collaboratively with Disability Support Workers, service delivery staff, families and healthcare professionals to promote safe, consistent and person-centred care. Rather than a traditional nursing management role, this position focuses on clinical guidance, capability building and ensuring appropriate clinical practices are followed across our services.

About the Company:

Enabling Pathways is committed to empowering individuals, fostering inclusion and supporting people to live meaningful lives within their communities.

We provide support across Community Access, Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA), with a strong focus on individual choice, dignity, wellbeing and quality of care.

As our services continue to support participants with increasingly complex needs, we recognise the importance of strong clinical guidance, well-trained staff and robust care practices.

About the Role:

As our Clinical Care Coordinator, you will provide clinical oversight and guidance across our disability support services, helping ensure participants' health needs are understood, documented and appropriately supported.

You will play a key role in developing staff capability, coordinating clinical training, reviewing health-related documentation and monitoring compliance with relevant care plans, protocols and procedures.

You will also work closely with our Service Delivery Coordinators to identify clinical risks, respond to changes in participant health and ensure concerns are escalated to the appropriate healthcare professionals.

The role will include visits to SIL properties, practical support and clinical guidance for staff, and direct nursing care when required and appropriate to your scope of practice.

We are looking for a Registered Nurse who combines sound clinical judgement with excellent communication skills, a collaborative approach and a genuine commitment to disability support.

Key Responsibilities:

Clinical Oversight and Participant Health

  • Provide clinical guidance across SIL, Community Access and other relevant disability support services.
  • Review participant health needs, clinical care plans and relevant health documentation to support safe and consistent care.
  • Support the development, implementation and review of clinical protocols, procedures and individualised health management plans.
  • Provide guidance to staff supporting participants with complex health needs and high-intensity daily personal activities.
  • Conduct visits to SIL properties to review clinical practices, identify risks and provide practical guidance to support workers.
  • Liaise with GPs, treating practitioners, allied health professionals and other healthcare providers to support coordinated care.
  • Provide direct nursing care when required, within your scope of practice and the arrangements in place for each participant.

Staff Training and Clinical Competency

  • Identify clinical training needs across the support workforce and coordinate relevant training and competency assessments.
  • Deliver or coordinate training in medication management, health monitoring, infection prevention, clinical documentation and other relevant care practices.
  • Coordinate specialised training for participant-specific needs, including PEG feeding, enteral care and other high-intensity support procedures, as required.
  • Assess, document and monitor staff competency in relevant clinical tasks, in accordance with applicable requirements and organisational procedures.
  • Provide practical coaching and guidance to Disability Support Workers to build confidence and promote safe care.
  • Work with external training providers and healthcare professionals where specialist instruction or assessment is required.
  • Maintain appropriate records of staff training, competency assessments and any required follow-up.

Clinical Documentation, Quality and Risk

  • Review medication records, health monitoring charts, clinical notes and other relevant documentation for completeness, accuracy and consistency.
  • Monitor whether clinical care plans, medication management protocols and relevant procedures are being followed.
  • Identify gaps in documentation, staff practice or clinical processes and work with relevant coordinators to implement corrective actions.
  • Contribute to clinical incident reviews, identify contributing factors and recommend improvements to reduce the risk of recurrence.
  • Support incident escalation, clinical risk assessment and appropriate follow-up when a participant's health or safety may be at risk.
  • Conduct clinical audits and checks across SIL properties and other relevant service settings.
  • Contribute to continuous improvement, policy development and audit readiness in relation to clinical care.
  • Maintain accurate, confidential clinical records in accordance with organisational requirements and relevant professional obligations.

Collaboration and Service Improvement

  • Work closely with Service Delivery Coordinators and senior management to identify clinical priorities and improve the quality of supports.
  • Provide clear, timely communication regarding clinical risks, training requirements and participant health concerns.
  • Support the development of practical systems, forms and procedures that improve clinical consistency across the organisation.
  • Participate in after-hours escalation arrangements as required, providing clinical advice within your professional scope and established escalation processes.

About You

You are a clinically capable and approachable Registered Nurse who enjoys supporting others to deliver safe, high-quality care. You can translate clinical knowledge into practical guidance that Disability Support Workers can understand and apply in their day-to-day work.

You will bring:

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Nursing experience and a sound understanding of clinical assessment, care planning, medication safety and clinical documentation.
  • The ability to identify clinical risks, recognise changes in participant health and determine when further assessment or escalation is required.
  • Strong communication skills and the ability to explain clinical procedures clearly to staff with varying levels of experience.
  • Experience, or a demonstrated ability to develop competency, in staff education, clinical training and assessment.
  • A commitment to evidence-informed practice, professional accountability and working within your scope of practice.
  • Strong organisational skills, attention to detail and the ability to manage competing priorities.
  • A collaborative approach to working with participants, families, support workers, coordinators and external healthcare professionals.
  • Confidence reviewing records, identifying gaps and following through on improvements.
  • Familiarity with the NDIS and disability support environment, or a genuine willingness to develop this knowledge.

Experience in disability services, complex care, medication management, PEG feeding, enteral care or other high-intensity support needs will be highly regarded.

We welcome Registered Nurses with transferable clinical experience who are interested in applying their skills in disability services and developing their knowledge of the NDIS environment.

Mandatory Requirements

  • Current registration as a Registered Nurse with AHPRA.
  • Proof of citizenship (passport, birth certificate or citizenship certificate) or VISA if applicable.
  • Evidence of Australian working rights.
  • Current QLD Open Driver's License.
  • Must hold a QLD Working with Children Blue Card.
  • NDIS Workers Screening Yellow Card.
  • Hold a current First Aid (HLTAID011) and CPR (HLTAID009) Certificate.
  • A clean National Police check less than 6 months old.
  • Completed NDIS Orientation Module Certificate.
  • Willingness to undertake additional training and maintain clinical competencies relevant to participant needs.

You must demonstrate a commitment to privacy, confidentiality, cultural awareness, professional conduct and the standards and requirements applicable to your nursing registration.
