Qualifications

• Experience supporting residents experiencing homelessness in residential or specialist service settings.
• Confident and compassionate communication with residents, carers, colleagues and stakeholders.
• Ability to work independently and as part of a dynamic team; develops and implements care plans.
• Comfort with diverse populations and strong record-keeping skills.

Responsibilities

• Provide 24/7 care, support, and supervision for residents.
• Collaborate with Homeless Health Service and partners to improve health outcomes.
• Assess residents, develop care plans, and refer to services as needed.
• Promote resident participation and integration with mainstream healthcare.
• Maintain accurate documentation within a harm-minimisation framework.
