Chosen Family is seeking an experienced and compassionate Registered Nurse with wound care expertise to provide ongoing, community-based nursing support to a 24-year-old female participant living in Berkshire Park, NSW.
This is an opportunity to use your clinical knowledge and professional skills to deliver consistent, person-centred wound management while helping a young woman maintain her comfort, dignity and quality of life in her own home.
The participant lives with Stage II Hidradenitis Suppurativa (HS), a chronic inflammatory skin condition that can cause recurrent abscesses, draining wounds, sinus tract formation and scarring. The condition affects her mobility and ability to independently manage personal care and wound care. During significant flare-ups, she may be bedbound for several days and require additional assistance.
We are looking for a nurse who understands that excellent wound care involves more than clinical technique. It requires patience, careful assessment, consistent documentation, sound clinical judgement and the ability to adapt care to a person's individual physical, sensory and communication needs.
If you are passionate about community nursing and want to build a meaningful, ongoing professional relationship with a participant, this opportunity may be an excellent fit.
The specific morning start time and commencement date will be confirmed. The ongoing schedule is subject to the participant's clinical care plan and confirmed service arrangements.
The participant has the following diagnosed conditions:
These conditions can influence her communication, sensory experiences, mobility, comfort and ability to participate in personal care.
We are seeking a nurse who can deliver clinically appropriate care while recognising the importance of predictability, respectful communication, consent, privacy and individual choice.
A neurodiversity-affirming and sensory-aware approach is essential. The successful applicant should be comfortable adjusting their communication and care delivery to the participant's needs, particularly during painful or physically challenging flare-ups.
You will be expected to support the participant's autonomy, explain clinical procedures in an accessible manner, respect her preferences and work collaboratively with her treating clinician and broader care team.
As the Registered Nurse providing this support, you will play an important role in maintaining continuity of care and monitoring the participant's wound health.
Your responsibilities will include:
Wound Assessment and Management
Clinical Monitoring and Escalation
Documentation and Continuity of Care
Person-Centred Community Nursing
We are particularly interested in Registered Nurses with practical experience in wound assessment, dressing selection and wound management.
Experience supporting people with chronic wounds, complex skin conditions, disability-related health needs or reduced mobility will be highly regarded.
The identified wound care products include:
The detailed wound management regimen, including dressing frequency, technique, product selection and any additional clinical recommendations, will be provided once available.
The successful nurse must review and clarify the relevant clinical instructions before commencing treatment. All wound care must be undertaken in accordance with the treating clinician's directions, applicable product guidance and the nurse's professional scope of practice.
To be considered for this opportunity, you should have:
Experience in community nursing, chronic wound management, complex care or supporting people with neurodevelopmental disabilities will be an advantage.
Applicable screening checks, certifications and other compliance requirements must be met before commencing where required for the role.
Consistent, meaningful community nursing
Build continuity into your nursing practice through recurring visits with a participant who benefits from reliable, individualised care.
A focused clinical role
Apply your wound care expertise through scheduled two-hour visits, with clear clinical responsibilities and care guided by an appropriate wound management plan.
Person-centred practice
Work in an environment that values the participant as an individual, not simply a clinical presentation. Your ability to combine technical competence with empathy and respect matters.
The opportunity to make a tangible difference
Your careful assessments, consistent dressing management and timely clinical escalation can contribute to the participant's comfort, skin health and overall wellbeing.
An inclusive professional community

We provide person-centred NDIS services that recognise individual strengths, preferences and goals. We are committed to fostering an inclusive, neurodiversity-affirming and culturally responsive environment where participants are supported to exercise choice, maintain dignity and live as independently as possible.
We value nurses who bring not only clinical expertise but also professionalism, accountability, compassion and a genuine commitment to improving the lives of the people they support.
