HomeCaring is seeking a skilled community nurse to provide direct clinical care in clients’ homes across Victoria. You will perform wound care, maintain concise documentation, and coordinate services with the care team to support client goals.

Ideal candidates have APRHA registration, CWCheck/NDIS checks or willingness to obtain, CPR within the last year, and strong English communication. A positive attitude and reliable work ethic are essential for success.

Competenze

• Current APRHA registration with zero notations.
• NDIS Worker Check (or willing to obtain).
• Working With Children’s Check (or willing to obtain).
• CPR (within one year).
• Excellent verbal and written English skills.
• A positive attitude and work ethic.
• Demonstrated experience in client assessment, goal-based care plan development, and enablement-focused care.

Key Responsibilities

• Provide and coordinate direct clinical care to our clients


• Clinical care skills, wound care and chronic wound management


• Manage client information through concise documentation that supports quality service delivery


• Working directly with a client in their home


• Working with a client with Down Syndrome and has hypoxic seizures


• Assist clients to reach their goals


• Current APRHA registration with zero notations


• Conducting clinical assessments


• CPR (within one year)


• Working With Children's Check (or willing to obtain)


• NDIS Worker Check (or willing to obtain)


• Excellent verbal and written English skills


• A positive attitude and work ethic


• Driving in the community

Ideally, our candidate will have

• Demonstrated experience in client assessment, goal-based care plan development, implementation of care services with an enablement focus, and client reviews.


• Concisedocumentation skills


• Excellent written and verbal communication skills


• Attention to detail and the ability to multitask


• Experience as a community nurse is a benefit for this role, but not essential.
