An exciting opportunity exists for a motivated and compassionate Registered Nurse to join the Keeping Well in Community Care Navigation/Care Coordination Team on a part-time basis (0.8 FTE).
As part of a multidisciplinary team, you will play a key role in supporting clients to navigate health and community services, promoting self-management, and ensuring holistic, patient-centred care. The successful applicant will demonstrate strong communication and organisational skills, a commitment to equity and quality improvement, and the ability to work collaboratively within a supportive team environment.
This is a rewarding role for a Registered Nurse who is passionate about making a difference in the community and supporting clients to achieve their health and wellbeing goals.
The primary purpose of this position is the assessment and screening of the needs of people with complex and chronic conditions. A key function of this assessment process is the determination and provision of further care intervention and support. The position will work to support patient enrolment, care planning, care navigation, health outcome monitoring and client education. This model is underpinned by the NSW Health Strategic Framework for Integrating Care. The Registered Nurse will provide safe, compassionate, quality, person-centred care and work in collaboration with the client, families/carers using a telephone-based service model.
