Job summary

Careply Pty Ltd in Sydney is seeking a Registered Nurse to deliver in-home clinical care and ongoing support. You will assess, plan and implement personalised care for clients in their own homes, working within a professional and supportive team.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Experience in Home Care, Community Nursing, Aged Care or similar clinical environment.
• Strong clinical assessment and care-planning skills.
• Experience in wound care, medication management and clinical risk management.
• Current Australian driver's licence and willingness to travel to clients.

Job description

As a Registered Nurse with Careply, you will provide clinical care, assessment and ongoing support to clients receiving services in their homes. This is a permanent full-time position offering the opportunity to provide high-quality, person-centred nursing care to clients in their own homes while working as part of a supportive and professional team.

Key responsibilities

• Conducting comprehensive nursing and clinical assessments
• Developing, reviewing and updating individualised care plans
• Providing wound care and ongoing wound assessment
• Medication management and administration
• Continence and catheter care where required
• Monitoring clients' health and responding appropriately to changes in clinical condition
• Providing clinical guidance and support to care and support workers
• Liaising with GPs, allied health professionals, hospitals, families and other healthcare providers
• Maintaining accurate and timely clinical documentation
• Supporting incident management and clinical follow-up

About you

• Current AHPRA registration as a Registered Nurse
• Experience in Home Care, Community Nursing, Aged Care or a similar clinical environment
• Strong clinical assessment and care-planning skills
• Experience in wound care, medication management and clinical risk management
• Excellent communication and interpersonal skills
• Strong documentation and organisational skills
• Ability to work independently and manage a community-based caseload
• Current Australian driver's licence and ability to travel to clients throughout the service area
• Current National Police Check and relevant employment screening requirements
• Permanent full-time employment – 38 hours per week
• Competitive salary package of $90,000 – $105,000 + superannuation, depending on experience
