Job summary

NSW Health invites applications for a Registered Nurse Discharge Planner at Maclean District Hospital. This permanent part-time role (24 hours per week) focuses on coordinating and delivering high-quality, person-centred nursing care in line with professional standards.

You will collaborate with the health care team to manage transfer of care and discharge planning for inpatients, supporting efficient patient flow and improved outcomes in the Northern NSW Local Health District.
