Job summary

Kurri Kurri Community Services (KKCS) is seeking a Registered Nurse Care Partner to join our Support at Home team. You will provide clinical oversight, assess complex needs and support the care journey in the community, working with ENs, families, GPs and allied health to ensure safe, coordinated care.

We welcome an experienced RN with AHPRA registration, aged care or community nursing experience, and a collaborative approach.

Qualifications

• Bachelor of Nursing or equivalent.
• Current AHPRA registration as a Registered Nurse.
• Experience within aged care and/or community nursing.
• Strong clinical assessment, care planning and risk management skills.
• Ability to meet KKCS pre-employment screening requirements.

Responsibilities

• Complete clinical assessments, care planning and reviews.
• Identify and manage clinical risks, deterioration and changing care needs.
• Provide clinical guidance to EN and Non-Clinical Care Partners.
• Coordinate complex care and safe delegation.
• Manage medication plans, incidents and clinical escalations.
• Support wound management, catheter care, enteral feeding and other complex nursing needs.
• Liaise with consumers, families, GPs, allied health and external providers.
• Maintain accurate clinical documentation and contribute to quality improvement.

Kurri Kurri Community Services (KKCS) is a local not-for-profit organisation delivering person centred services across the Hunter.
Hunter Recruitment Group is partnering with KKCS to recruit a Registered Nurse Care Partner to join their growing Support at Home team.

This role is an opportunity for an experienced RN to combine community nursing, complex care and clinical leadership, supporting older people to live safely and independently at home. The position has been created to support growth and increasing clinical needs within the service.

About the Role

Working within KKCS's pod model, you'll provide clinical oversight and guidance to Enrolled Nurse and Non-Clinical Care Partners while remaining hands on with consumers in the community.
You'll manage complex clinical needs across the consumer journey, from intake and assessment through to care planning, reviews, risk management and responding to changes in health.
You'll also work closely with families, GPs, allied health professionals and other providers to ensure care remains safe, coordinated and responsive.

What You'll Be Doing

About You

You'll be an experienced RN who is comfortable making clinical decisions, managing complex needs and working autonomously in the community, while being part of a supportive team.

You'll bring:

• Bachelor of Nursing or equivalent
• Current AHPRA registration as a Registered Nurse
• Experience within aged care and/or community nursing
• Strong clinical assessment, care planning and risk management skills
• Confidence supporting consumers with complex or changing health needs
• Strong communication and clinical documentation skills
• Current Driver's Licence and reliable, comprehensively insured vehicle
• Current First Aid and CPR
• Ability to meet KKCS pre-employment screening requirements

Highly regarded:

• Support at Home or Home Care Package experience
• Care Partner or case management experience
• Urethral and suprapubic catheter competency, or willingness to obtain
