Job summary

Kurri Kurri Community Services (KKCS) is seeking a Non-Clinician Care Partner to join the Support at Home team in Kurri Kurri. The role focuses on coordinating services for older people living at home, working with Enrolled Nurse Care Partners and under RN clinical oversight, with occasional consumer visits and onboardings.

Kurri Kurri Community Services (KKCS) is a local not-for-profit organisation delivering person centred services across the Hunter.

Hunter Recruitment Group is partnering with KKCS to recruit a Non-Clinician Care Partner to join their Support at Home team.

This is a great opportunity for someone with experience across aged care, community services, care coordination or Home Care Packages/Support at Home who enjoys the organisation, relationships and problem-solving behind coordinating quality care.

About the Role

KKCS has recently introduced a collaborative pod model within their Support at Home team.

As a Non-Clinician Care Partner, you'll work side-by-side with an Enrolled Nurse Care Partner, supported by Registered Nurse clinical oversight, to coordinate services for a shared group of older people living within the community.

Your focus will sit predominantly across the coordination and administration of care, keeping services organised, monitoring budgets, maintaining documentation, communicating with consumers and families and ensuring each person's supports continue to meet their individual needs and goals.

While predominantly office based, you'll also have opportunities to meet consumers in their homes, assist with onboarding and complete reviews.

What You’ll Be Doing

• Coordinate services and supports under the Support at Home program

About You

• Certificate III in Individual Support (Ageing) or equivalent
• Experience within aged care, community care, care coordination or a similar environment
• Strong organisation and administrative skills
• Confidence managing documentation, budgets and competing priorities
• Strong communication and relationship-building skills
• Confidence using client management systems and general computer applications
• Current Driver's Licence and access to a reliable, registered and comprehensively insured vehicle
• Current First Aid and CPR
• Ability to meet KKCS pre-employment screening requirements

Highly regarded

• Previous Home Care Package or Support at Home experience
• Previous Care Partner/Care Coordinator experience
• Knowledge of aged care funding, budgets and quality requirements
