Job summary

Amplar Health, part of the Medibank Group, is transforming in-home care and community-based programs, seeking a compassionate Registered Nurse to deliver person-centred care at home.

You will work autonomously with Clinical Care Partners and allied health, focusing on prevention, early intervention and reablement for older Australians, delivering coordinated care across the care journey.

Qualifications

• Current unrestricted AHPRA registration as a Registered Nurse.
• Minimum 2 years' nursing experience in aged care and community settings.
• Strong clinical assessment and care planning skills within aged care.
• Live in or near North Gold Coast areas such as Arundle.
• Excellent communication and relationship-building abilities.

We're redefining healthcare by bringing As part of Amplar Health's growing Support at Home service, you'll be working within a contemporary, clinically-led model designed to help older Australians remain independent, mobile and connected in their own homes for longer. Our approach is centred on prevention, early intervention and reablement, helping clients maintain function and quality of life rather than simply responding when health issues arise.

You'll work alongside Clinical Care Partners, other allied health and nursing professionals, as well as Amplar Health's broader primary care network to deliver coordinated, person-centred care. This integrated approach provides a unique opportunity to influence health outcomes across the entire care journey and make a meaningful difference to how older Australians experience ageing at home.
