Metro South Health in Brisbane is seeking a Registered Nurse in the Community Services Team to deliver person-centred nursing care in clients' homes and community settings, including Hospital in the Home and MAPS programs.
You will work with multidisciplinary teams to assess, plan, deliver and evaluate care, with increasing autonomy, contributing to hospital avoidance and positive health outcomes.
Deliver meaningful, person-centred care where it matters most—supporting community clients in their homes and clinic settings across acute and post-acute care pathways.
As a Registered Nurse in the Community Services Team, you'll provide safe, compassionate and evidence-based nursing care to clients across Hospital in the Home (HITH), GEMRHITH and Multidisciplinary Avoidance and Post-Acute Service (MAPS) programs. This is an opportunity to build your clinical expertise, work with increasing autonomy and make a meaningful difference to people in their homes and community.
You're a compassionate and confident Registered Nurse who thrives in a community setting, bringing strong clinical judgement, collaborative practice and a commitment to delivering person-centred care that supports positive outcomes.
