Job summary

Central Adelaide Local Health Network is seeking a Pharmacist for Mt Barker to provide a broad range of professional and clinical pharmacy services, optimise medication therapy and support safe patient transitions across Mt Barker Hospital and spoke sites.

The role is a temporary part-time position (0.6 FTE) based in Adelaide Hills with both on-site and remote service delivery. Applicants must hold Australian pharmacist registration (AHPRA) and demonstrate collaboration with healthcare teams.

Qualifications

• Essential qualifications, registration and selection criteria must be completed once the AHP2 Pharmacist role description is supplied.
• Ability to provide a broad range of professional and clinical pharmacy services to support safe medication management.
• Proven ability to collaborate with medical, nursing and multidisciplinary teams to optimise patient outcomes.

Responsibilities

• Provide a broad range of professional and clinical pharmacy services for inpatients and outpatients.
• Optimise medication therapy and provide expert medicines advice.
• Work with multidisciplinary teams to support safe transitions of care.
• Deliver on-site and remote services as required for Mt Barker Hospital and spoke sites.
