Job summary

Eyre and Far North Local Health Network (EFNLHN) is seeking an Advanced Clinical Practitioner - Regional Complex Care to lead specialised acute care and disability services across our regional health network.

With a focus on complex discharge planning and patient flow, you will work autonomously with multidisciplinary teams to improve outcomes. This role offers clinical leadership, education, research and ongoing development of best-practice models of care.

Qualifications

• Must hold a recognised allied health qualification within Occupational Therapy and be registered with AHPRA.
• Experience in complex clinical practice and leadership is desirable.

Responsibilities

• Lead expert clinical services for consumers with complex care, disability and rehabilitation needs.
• Provide advanced clinical leadership to multidisciplinary teams, supporting assessment and discharge planning.
• Drive patient flow, safe care pathways and coordinated discharge processes.
• Lead service development, education, quality improvement and research.
• Provide strategic advice to senior leadership using data and outcomes.
