Job summary

Croydon Health Services NHS Trust is seeking an Associate Practitioner to join the Rapid Response/Virtual Ward. The post holder will work to enable unwell patients to remain at home with the help of technology and will perform delegated clinical tasks under supervision where required.

They will support Occupational Therapists, Physiotherapists and Rapid Response Matrons, working autonomously across home and community settings as needed in Croydon and surrounding areas.

Qualifications

• Associate Practitioner competencies define the boundaries for delegated clinical tasks.
• Proficient to work at a higher competency level than a Band 3 Health Care Support Worker.
• Work in partnership with team members, service users, general practice, multidisciplinary team and private/voluntary agencies.
• Provide skilled support to the Occupational Therapists, Physiotherapists and Rapid Response Matrons.
• Work autonomously in the patient’s own home and other community settings.

Responsibilities

• Facilitate the work of the Rapid Response/Virtual Ward to enable unwell patients to remain at home and achieve their full potential with technology.
• Carry out delegated clinical tasks and responsibilities as defined by registered nurses or therapists.
• Support the team in community settings including home visits, intermediate care beds and care homes as required.
