Job summary

Goulburn Valley Health is seeking a Project Manager to lead a quality improvement initiative within the Transition Care Program. You will coordinate data collection, analysis and stakeholder engagement to improve occupancy, referrals and client access across TCP services.

The role requires collaboration with internal and external partners to identify barriers, develop evidence-based recommendations, and support implementation of improved service models to strengthen patient flow and service

Responsibilities

• Lead and deliver a quality improvement project examining factors influencing engagement, referral activity and occupancy outcomes within Residential Transition Care Program (TCP) services across GV Health brokered residential care providers.
• Identify barriers and enablers impacting TCP utilisation and develop evidence-based recommendations to improve occupancy and client access to TCP services.
• Collaborate with internal and external stakeholders to collect, analyse and interpret quantitative and qualitative data and support implementation of improved service models.
• Contribute to service redesign and future planning to ensure TCP services remain client-centred, responsive and financially sustainable.

The Project Manager is responsible for leading and delivering a quality improvement project examining factors influencing engagement, referral activity and occupancy outcomes within Residential Transition Care Program (TCP) services across GV Health brokered residential care providers.

The role will work collaboratively with internal and external stakeholders to collect, analyse and interpret quantitative and qualitative data and support implementation of improved service models.

The position will contribute to service redesign and future planning to ensure residential TCP services remain client-centred, responsive, financially sustainable and aligned with system demand.
