Job summary

Binbi Yadubay Residential Rehabilitation Service, operated by Lives Lived Well, north of Rockhampton, is seeking a Pre and Post Worker to support clients entering and exiting the residential program. You will conduct assessments, facilitate group work, and develop recovery-focused plans to strengthen readiness for treatment and long-term recovery.

Qualifications

• Experience in client support and case management.
• Knowledge of trauma-informed practice and person-centred approaches.
• Strong assessment and risk management skills.

Responsibilities

• Conduct client assessments and risk assessments.
• Support individuals entering residential treatment.
• Deliver individual and group-based interventions.
• Develop recovery-focused support plans.
• Coordinate referrals and access to community services.
• Provide transition planning and post-program support.
• Maintain accurate case notes and records.
• Collaborate with clinical and residential teams to support outcomes.
