Plena Healthcare is seeking a qualified Physiotherapist to join our multidisciplinary team in a hybrid role spanning Residential Aged Care and Community services in Adelaide North. You will deliver evidence-based physiotherapy care to older adults to maintain mobility and independence, supported by a network of allied health peers.

The role is part-time (4 days a week) with openness to full-time, servicing RAC facilities and private homes in Adelaide's Northern Suburbs.

Qualifications

• Bachelor’s degree in Physiotherapy or equivalent.
• Current AHPRA registration.
• Australian working rights.
• Valid NDIS Worker Screening Check and National Police Check (or willingness to obtain).

Responsibilities

• Clinical Assessments: Conduct mobility, transfers, fall prevention, skin integrity, equipment needs, and pain management assessments.
• Residential Aged Care: Deliver individual therapy, mobility and functional programs, care plan reviews, and group-based sessions.
• Community Services: Provide care to clients at home under Home Care Packages (HCP), STRC, CHSP, and private funding.
• KPI Management: Achieve reasonable billable targets while maintaining quality patient outcomes.
• Collaboration: Work with clients, families, facility staff and multidisciplinary colleagues to meet goals.

Physiotherapist – Residential Aged Care & Community - Adelaide North

About the Role

Plena Healthcare is seeking a qualified Physiotherapist to join our multidisciplinary team in a hybrid role spanning Residential Aged Care and Community services.

In this position, you will deliver evidence-based, person-centred physiotherapy care that helps older adults maintain their mobility, functional independence, and quality of life. You will be part of an innovative, collaborative model of allied health service delivery supported by experienced peers across Occupational Therapy, Podiatry, Speech Pathology, and Dietetics.

• Servicing RAC facilities and private homes within Adelaide's Northern Suburbs

Key Responsibilities

• Clinical Assessments: Conduct comprehensive assessments covering mobility and transfers, fall prevention, pressure care, skin integrity, equipment needs, and pain management.
• Residential Aged Care: Deliver individual therapy interventions, mobility and functional programs, care plan reviews, and group-based active ageing sessions.
• Community Services: Provide tailored care to clients living independently under Home Care Packages (HCP), Short Term Restorative Care (STRC), CHSP, and private funding.
• KPI Management: Work within reasonable, achievable billable targets designed to support quality patient outcomes without burnt‑out schedules.
• Collaboration: Partner with clients, families, residential facility staff, and multidisciplinary colleagues to achieve meaningful goals.
