Job summary

Plena Healthcare is seeking a Physiotherapist to join our Residential and Community Aged Care team in Sydney. You will deliver personalised, evidence-based therapy programs with a focus on mobility, function, and active ageing, working within a multidisciplinary team.

In this role, you will conduct care plan reviews, provide one-to-one interventions, and deliver group-based activities across RAC and community settings.

Qualifications

• General AHPRA registration is required and current.
• Current Police Check or willingness to obtain.
• Current driver’s licence and access to a vehicle.
• Willingness to work in a mobile capacity within a dedicated region.
• NDIS Worker Screening Check or willingness to obtain.

Responsibilities

• Deliver personalised, evidence-based therapy programs including mobility and functional work.
• Conduct care plan reviews for residents.
• Provide one-to-one interventions and group-based active ageing activities.
• Collaborate with a multidisciplinary team in RAC and community settings.
