Job summary

UnitingCare Health Pty Ltd. in Cooroy/Gympie is seeking a Physiotherapist to join Blue Care’s community team.

You will provide holistic, integrated therapy to support mobility, independence and wellbeing across Cooroy and Gympie regions, tailoring care to improve clients’ quality of life. You will develop new treatment programs, manage movement and pain, and liaise with GPs and other professionals to ensure comprehensive care plans.

Qualifications

• Tertiary qualifications and current AHPRA registration required.
• Experience in aged care or community setting preferred.
• Strong communication and relationship-building skills.
• Good time-management and clinical problem-solving abilities.
• Current Australian Driver's Licence and vehicle with KM reimbursement is essential.
• Current CPR and First Aid certification.

Responsibilities

• Diagnose, assess and develop care plans for individuals and groups.
• Prescribe and select appropriate adaptive equipment and modifications.
• Educate clients and carers on therapy aspects.
• Set goals and manage discharge planning and resolution.
• Coordinate with GPs, nurses and occupational therapists to ensure aligned care plans.

Your part in our Blue Care team

You'llprovide holistic, integrated care to our clientsenhancing their mobility and movement to promote their independence and wellbeing. Working with a number of movement conditions associated with musculoskeletal impairments, medical conditions and chronic disease, you will tailor your care tomake a real difference in your client's quality of life!

You'll have the opportunity to develop new treatment programs, helping clients to manage their movement, pain, balance and manual handling.This will include:

• Diagnosing, assessing and developing care plans for individuals and groups
• Prescription and selection of appropriate adaptive equipment and modification
• Education and training on aspects of therapy for the clients and carers
• Goal setting activities, as well discharge and resolution planning
• Liaising with healthcare professionals, such as GPs, nurses and occupational therapists to ensure care plans are formulated and monitored

• Current Australian Drivers Licence and a road worthy vehicle with comprehensive insurance as required - Essential
• Confidence to satisfy the relevant probity checks required by legislation or policies
• Current CPR and First Aid
• Own reliable vehicle with kilometre reimbursement where a fleet car is not available
