Accent Home Care is seeking a Physiotherapist to join our fast-growing Home Care team in Melbourne. The role involves delivering physiotherapy services in participants' homes, focusing on elderly care, with opportunities for full-time or part-time work.

You will conduct home visits, develop evidence-based treatment plans, and collaborate with families, caregivers, and multidisciplinary teams to optimise functional outcomes.

Qualifications

Seeking a Physiotherapist to join our fast growing and culturally diverse Home Care company.

Accent Home Care is seeking aPhysiotherapist who will have the opportunity to work either full-time or part-time as part of ourClinical Home Care Team. The successful therapist will be passionate about caring for the elderly and is seeking a long and rewarding career with Accent Home Care.

Key responsibilities

• Managing referrals for Physiotherapy services
• Conducting home visits to participants predominately in the Southern and Eastern areas of Melbourne
• Assess and develop appropriate treatment programs for participants, engaging with their families/representatives and co-workers to ensure best possible treatment is given.
• Assessment and prescription of assistive aids and equipment for resident and participants rehabilitation needs.
• Provide goal-based intervention based on assessed needs and care plans.
• Liaise with medical, nursing, and allied health agencies regarding physiotherapy intervention.
• Deliver Support at Home services, including Restorative Care and End-of-Life pathways

For this position you will have:

• Current registration as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA).
• Member of the Australian Physiotherapy Association or have alternate personal indemnity insurance (or be willing to obtain).
• Previous experience working with older adults including their families and carers.
• Ability to prescribe assistive equipment based on functional assessed need.
• Well-developed communication, planning, organisational and time management skills.
• Concise report writing and documentation skills.
• Proven ability to establish and maintain effective and cooperative internal and external working relationships.
• Strong IT skills and the ability to use databases.
• Ability to create measurable goals based on achievable outcomes for consumers.
• Ability to work independently and as part of a multidisciplinary team.
• An understanding of aged-specific diagnoses and relevant interventions.
• Excellent people skills and a genuine passion for caring for the elderly.
• Australian Citizenship or Permanent Residency.
• A valid police clearance (Less than one year old)
• A current driver’s licence and a motor vehicle.
