Job summary

My Community Health is seeking a dedicated Community Physiotherapist to join our in-home nursing team servicing the Central Coast regions. You will assess, plan and deliver physiotherapy services tailored to individual clients, coordinating with families and care teams to support independence and quality of life.

This Monday–Friday role offers no shift work, a supportive, collaborative culture and opportunities to grow into team-leader positions as our services expand.

Qualifications

  • Tertiary qualification in Physiotherapist and current registration with AHPRA.
  • Excellent communication with clients, families and multidisciplinary teams.
  • Strong clinical reasoning and problem-solving with evidence-based approach.
  • Passion for promoting health, wellbeing and independence in the community.
  • Flexible and collaborative team player.
  • Australian working rights.

Responsibilities

  • Conduct thorough initial assessments to develop customised treatment plans for each client.
  • Deliver a range of physiotherapy interventions, including manual therapy, exercise prescription, and rehabilitation programs.
  • Collaborate with clients, their families, and other healthcare professionals to ensure seamless, coordinated care.
  • Maintain accurate and up-to-date client records, ensuring compliance with all relevant policies and regulations.
  • Participate in regular team meetings and professional development opportunities to stay informed of industry best practices.
  • Champion a client-centred approach, advocating for the unique needs and goals of each individual.

  • Excellent clinical reasoning and problem-solving skills, with a commitment to evidence-based practice

  • Passion for promoting health, wellbeing, and independence in the local community

  • Flexible and adaptable approach, with the ability to work collaboratively in a team environment

  • Must have Australian working rights
