About the role

We are currently looking to add passionate and dedicated Personal Care Workers to our professional care team at our Andrew Kerr Care Facility to work a Wednesday to Saturday 21:45pm to 07:15am Roster.

At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents’ conditions and escalate observations and concerns.

In this role, you will:

  • Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
  • Provide personal care to residents with complex needs such as palliative care, dementia support, restorative care and pain management to enhance their wellbeing
  • Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
  • Monitor the cleanliness, safety, environment and promptly report issues of concern
  • Part-Time opportunity working at our Andrew Kerr Care Facility in Mornington
About you

This is the place for you if you have:

  • Certificate III in Individual Support, or equivalent (Aged Care)
  • Experience in residential aged care/ home care (desired)
  • Experience with dementia care and palliative care support (desired)
  • Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners
