Job summary

Uprety Home Care Pty Ltd is seeking an experienced Community Registered Nurse for permanent part-time roles across Lake Macquarie and Newcastle. You will visit clients at home, assess nursing needs, and develop and review individualized care plans while delivering care within your scope of practice.

You will balance direct client care with clinical documentation, assessment reports, and follow-up, coordinating with administration and other healthcare professionals.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• At least five years of community nursing experience or demonstrated readiness to work independently.
• Experience in complex wound care, SPC/IDC catheter management, and continence care.

Responsibilities

• Complete nursing assessments and develop person-centred care plans.
• Provide wound assessments and catheter care, including SPC and IDC changes where appropriate.
• Complete continence assessments and prepare clear reports.
• Deliver nursing care to NDIS, aged-care and DVA clients.
• Maintain accurate progress notes, care plans and clinical records.
• Travel between clients within the service area and coordinate workload.
• Educate clients and carers about care requirements.
• Plan workload, meet deadlines and communicate barriers to timely completion.
