Job summary

Optimum Health Solutions – Croydon Park NSW is seeking a Registered Nurse to deliver in-home care to NDIS participants across Sydney's south and inner west. The role starts part-time with a clear pathway to full-time as caseload grows, and you will travel between visits, working independently in the community rather than a clinic.

The job focuses on wound care, catheter management, PEG feeding, and medication administration within a relationship-based care model.

Qualifications

• Current AHPRA registration without conditions.
• Minimum 3 years' post-graduate nursing experience.
• NDIS Worker Screening Check or willingness to obtain.
• National Police Check or willingness to obtain.
• NSW Working With Children Check or willingness to obtain.
• NDIS Worker Orientation Module completed before starting.
• Current CPR and First Aid certification.

Responsibilities

• Provide wound care and pressure injury management.
• Undertake catheter insertion and management (IDC/SPC).
• Support PEG feeding and enteral nutrition.
• Administer medications.
• Conduct continence assessments and product recommendations.
