Job summary

Bayside Health, Caulfield Community Health Service, is seeking a Grade 2 Occupational Therapist for a fixed-term, part-time role (2 days per week) through to June 2027. Located at Caulfield Hospital, this position offers salary packaging and generous wellbeing opportunities within a multi-disciplinary team.

You will assess, plan, and deliver OT services to home-based clients, liaising with agencies to coordinate care and home modifications, with a focus on independence and safety for clients

Qualifications

• Degree in Occupational Therapy.
• Minimum 3 years’ experience in community health.
• AHPRA registration and ongoing professional development requirements.
• Current Victorian driver's licence.

Responsibilities

• Provide comprehensive home-based assessments and OT services.
• Develop and implement person-centred care plans.
• Collaborate with a multidisciplinary team to support clients.
• Coordinate with local councils and contractors for home modifications.
• Advocate for clients with complex care needs (e.g., My Aged Care, CHSP).
