Job summary

Hunter New England Local Health District is seeking a Registered Nurse to join Bulahdelah Community Health Service. You will deliver wound management and palliative care in clinic and home settings as part of a supportive multidisciplinary team.

You will work with a Nurse Unit Manager and specialists, with opportunities for education and professional development, flexible part-time hours, and salary packaging options.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Experience with wound physiology and wound healing.
• Experience in wound management and/or palliative care would be highly regarded.
• Strong clinical assessment, critical thinking and decision-making skills.
• Ability to prioritise and coordinate a caseload in the community.

Responsibilities

• Provide high-quality nursing care to patients in homes and community clinic.
• Assess, manage and monitor wounds across settings.
• Apply wound physiology knowledge to clinical practice.
• Provide compassionate, person-centred palliative care to patients and families.
• Develop and implement individualised care plans with patients and teams.
• Coordinate and prioritise caseload to ensure safe, timely care.
• Work with Palliative Care team and Wound Management Specialist.
• Educate and support patients and families to promote independence and quality of life.
• Maintain accurate clinical documentation and communicate with healthcare providers.
• Contribute to a collaborative team environment and ongoing practice improvement.
