Job summary

Barwon Health is seeking a Clinical Pharmacist Grade 1 to provide clinical and operational pharmacy support at the McKellar Centre, with a focus on sub-acute services such as rehabilitation, palliative care and geriatric pharmacy. The role aims to support safe, appropriate and efficient medication use and patient flow across hospital and community settings.

You will work with multidisciplinary teams to deliver medication reconciliation, medicines management, patient counselling and discharge

Qualifications

• Registered Pharmacist with AHPRA and hospital or clinical pharmacy experience, preferably within sub-acute or inpatient settings.
• Strong knowledge of pharmacotherapy and the ability to support safe, effective and evidence-based medication management.
• Excellent communication and teamwork skills with the ability to work collaboratively across multidisciplinary teams.
• Commitment to ongoing professional development and education, quality improvement and patient-centred care.

Responsibilities

• Deliver clinical pharmacy services including medication reconciliation, medicines information, patient counselling and discharge planning across sub-acute and inpatient services.
• Support safe and cost-effective medication use through formulary compliance, antimicrobial stewardship, restricted medicine management and medication safety activities.
• Collaborate with medical, nursing, allied health and pharmacy teams to optimise patient care and ensure timely medication management.
• Contribute to education, service delivery, quality improvement and professional development while supporting students, interns and pharmacy technicians.
