Job summary

St Vincent’s Hospital Sydney is seeking a Social Worker (Level 1-2) for a max-term full-time contract until June 2027. The role supports both inpatient and community palliative care, operating within a multidisciplinary team to provide person-centred end-of-life care.

The position involves on-call crisis coverage, psychosocial assessments, care coordination, counselling, and advocacy for patients and families facing complex needs, with ongoing professional development and quality improvement

Qualifications

• Experience in healthcare settings, especially with palliative care, end-of-life or chronic illness.
• Strong skills in psychosocial assessment, casework, counselling and advocacy.
• Ability to work effectively in multidisciplinary teams and with patients’ families

Responsibilities

• Provide social work interventions as part of a multidisciplinary team delivering person-centred end-of-life care.
• Participate in 24-hour on-call social work crisis service outside regular hours.
• Undertake psychosocial and risk assessments addressing emotional, spiritual, social and practical needs.
• Support care coordination and end-of-life planning with information and access to services.
• Provide therapeutic counselling, advocacy and support to patients, families and carers.
• Engage in professional development, team learning and quality improvement initiatives

Education

Tertiary qualifications in Social Work, eligible for AASW membership and Full Class C Drivers License
