Job summary

South Western Sydney Local Health District is seeking a Palliative Care Clinical Nurse Specialist to join Rosemeadow Community Health Centre. The CNS will lead education, mentoring and clinical guidance for clients, carers, and staff, supporting a multidisciplinary team to deliver high-quality palliative care.

The role requires AHPRA-registered nursing, post-registration qualifications, and at least 3 years of relevant clinical experience.

Qualifications

• AHPRA registration as a Registered Nurse is essential.
• Post-registration qualifications in nursing or specialist areas are preferred.
• Minimum 3 years' experience in relevant clinical areas.
• Strong communication and multidisciplinary teamwork abilities.
• Demonstrated risk management and quality improvement experience.

Responsibilities

• Provide ongoing education, mentoring and clinical support to clients, carers and staff.
• Support and guide Community Health Nurses and multidisciplinary teams in palliative care.
• Promote patient-centred care and evidence-based nursing practices.
