Illawarra Shoalhaven Local Health District is seeking a Clinical Nurse Specialist Grade 2 (CNS 2) to deliver high-quality palliative care in the community. You will work within the ISLHD Community Palliative Care Service, providing direct patient care and coordinating with primary care and other services.

The CNS 2 will operate with minimal direct supervision, demonstrating leadership and delivering evidence-based nursing care across the defined population.

Formación

• Current AHPRA Registration with relevant post-registration qualification.
• At least 3 years' experience in the clinical area of their post-graduate qualification.
• Palliative care experience is highly desirable.

Responsabilidades

• Provide patient-centred palliative care in the home environment within a multidisciplinary team.
• Perform triage, assessment and ongoing care planning.
• Lead and supervise junior clinicians or community health nurses as required.
