Job summary

Wesley Mission Queensland Limited is seeking a Family & Community Support Practitioner for Hummingbird House. In this part-time role (30 hours/week) you will provide psychosocial and emotional support to children and families facing serious illness, end-of-life care and bereavement, within a multidisciplinary team and family-centred care framework.

You will conduct assessments, develop tailored plans, deliver grief and bereavement support and collaborate with healthcare and community partners to

Qualifications

• Completed tertiary qualification in Social Work and eligibility for AASW Membership.

Responsibilities

• Conduct psychosocial assessments and tailor support plans.
• Provide emotional, psychosocial, grief and bereavement support to children and families.
• Support families during crisis, distress and clinical events.
• Facilitate grief and bereavement groups and memory-making activities.
• Collaborate with multidisciplinary teams to deliver family-centred care.
• Build relationships with healthcare providers, schools and community organisations.
