Job summary

Catholic Healthcare invites an Occupational Therapist to join our community aged care team in the Northern Sydney area. This role offers a varied, autonomous practice with travel between clients and dedicated time for administration and reporting.

We value commitment to clients, families and carers, with a focus on person-centred and reablement approaches. AHPRA registration is required and sponsorship for PR may be available.

Qualifications

• Tertiary qualification in Occupational Therapy.
• Current AHPRA registration.
• Experience with equipment prescription, home modifications and falls prevention for older adults.
• Knowledge of wellness and reablement models of care in community health.

Responsibilities

• Conduct comprehensive OT assessments for clients, including those with complex needs.
• Provide OT services including home assessments, ADL recommendations, home modifications and equipment prescriptions.
• Develop and facilitate community health and wellness programs.
• Involve clients and carers in person-centered care plans and reablement strategies.
• Support and train community workers and staff to best assist clients.
• Submit applications for home modifications and equipment per program guidelines.
• Collaborate with nurses, physiotherapists, and allied health staff.
• Engage in face-to-face clinical work with travel between clients and time for admin.

This is a unique opportunity for an Occupational Therapist looking for a varied and autonomous role in Community Aged Care on a Full-time or Part-time permanent basis.

Day To Day This Role Looks Like

• Conduct comprehensive OT assessments for clients, including those with multiple and complex needs.
• Provide specific OT services and programs, including home assessments and advice on activities of daily living with recommendations involving home modifications, equipment prescription, care planning and education
• Facilitate and assist in development of a range of community based Health and Wellness programs and groups
• Involve clients and carers in developing care plans that are person centred and reablement focused.
• Provide support, education and direction to community workers and staff to assist them to best support our clients
• Submit individual applications for home modifications and equipment in accordance with program guidelines
• Work collaboratively with the wider allied health team, including nurses, physiotherapists, fitness instructors and allied health assistants
• Face to face clinical work and minimum travel time between clients with time allocated for administration and reporting
• Ongoing liaison with external agencies and suppliers to arrange services and products required by the clients in accordance with policies and procedures

To Be Successful In This Role You’ll Need

• Tertiary qualification in Occupational Therapy with post graduate experience
• Current AHPRA registration
• Commitment and enthusiastic approach towards working with the clients, families, carers and advocates.
• Experience in equipment prescription, pressure care, home modifications and falls prevention for older adults
• Industry knowledge of local and international wellness and reablement models of care
• Experience working in a community setting and demonstrated knowledge of the aged and disability service system in the community health sector - desirable but not essential
• Current Drivers Licence and registered and insured motor vehicle
• Willingness to apply for a Police Check
