Job summary

Barwon Health invites applications for a Permanent Part Time Nurse Practitioner role in Geelong, Australia. Join a supportive multi-disciplinary team providing in-reach care to residents of aged care facilities, aiming to avoid unnecessary hospital presentations.

20 hours per fortnight with pro-rata five weeks leave and salary packaging options. In this role you will triage, assess deteriorating residents, develop care plans, follow up, and educate staff and families, supported by geriatricians,

Qualifications

• Authorised to practice as a Nurse Practitioner and Registered Nurse with AHPRA and able to meet ANMC competencies for the Registered Nurse.
• Information Technology skills including Microsoft Office, Outlook and Patient Management Systems.
• Excellent time management and organisational skills.
• Evidence of a focus on and a passion for high quality patient care.

About the role

Residential In-Reach (RIR)providesan alternative to the emergency department where clinicallyappropriateand safe, for people living in residential aged care facilities.

Become part of a specialist multi-disciplinary team responsible for assessment and management of resident’s acute medical condition within the aged care facility in collaboration with the resident and their representatives to avoid unnecessary hospital presentations.

Supported by Geriatricians, Registrar, Nurse Practitioner and Registered Nurses this is a unique role that provides specialist care to residents of residential aged care facilities.

Whatwe’relooking for

• Authorised to practice as a Nurse Practitioner and Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) together with demonstrated ability to meet the ANMC competencies for the Registered Nurse

• Excellent time management and organisational skills
