Job summary

Barwon Health in Geelong is seeking a Nurse Practitioner to join a specialist residential in-reach team supporting aged care facilities. You will assess acute conditions, plan care with residents and families, and collaborate with geriatricians, nurses and allied clinicians.

The role includes education of staff and families, in a supportive multi-disciplinary environment, with access to ongoing professional development and salary packaging benefits.

Qualifications

• Authorised to practice as a Nurse Practitioner and Registered Nurse with AHPRA.
• Demonstrated ability to meet the ANMC competencies for the RN.
• IT skills including Microsoft Office, Outlook and patient management systems.
• Excellent time management and organisational skills.
• Evidence of a focus on and a passion for high quality patient care.

About the role

Residential In-Reach (RIR)providesan alternative to the emergency department where clinicallyappropriateand safe, for people living in residential aged care facilities.

Become part of a specialist multi-disciplinary team responsible for assessment and management of resident’s acute medical condition within the aged care facility in collaboration with the resident and their representatives to avoid unnecessary hospital presentations.

Supported by Geriatricians, Registrar, Nurse Practitioner and Registered Nurses this is a unique role that provides specialist care to residents of residential aged care facilities.
